This is an antique hot water bottle cover, possibly, purchased in Italy. There are two stains and one repaired tear on the inside, due to its antique nature. I haven't used it, so I'm not sure how to use it, but since it's not bag-shaped, I think you would use it by sandwiching the hot water bottle between the top and bottom fabric.
