This is S.M.H. No. 16, published in January 2000. I purchased it at a bookstore when it was first published. I have looked through it many times since then and used it as a reference for work, so the condition is poor. In particular, the left side of the cover is discolored from sun exposure, with the original yellow fading to almost white. There are also wrinkles and scratches, and the glue is weakening, so the first page on the cover side has come loose, although the pages themselves are still bound.
