The shoulder and waist straps have quilt batting inside. Since children will be carrying or wearing the dolls on their backs, the part where the doll goes is made into pants with elastic at the waist. For back carrying, it's like putting the doll in the pants and wearing a bag. There's also a pocket. The shoulder straps are adjustable with adjusters, and the waist is adjustable with Velcro. There is a buckle on the chest to prevent the shoulder straps from slipping. My 1 year and 9 month old daughter was able to click it in. Unfortunately, she couldn't take it off.
