The parts in photos 11 and 12 were included in the box, but I don't know where they attach. The light bulb works for now. Assembly instructions are printed on the underside of the top lid. I was able to assemble it, and I've never assembled one before, so I don't think it's very difficult. The item will be packaged by wrapping the entire box in bubble wrap, then waterproofing it with plastic, and shipped without a cardboard box. (Fragile and "Do Not Stack" stickers will be applied.)
