This is a picture book, "O-toire-san" (Mr. Toilet), by Megumi Kitagawa. With cute illustrations and a unique story, this book makes toilet training fun. It's recommended not only for children during toilet training but also for reading aloud at nursery schools and kindergartens. I purchased this book a few years ago and have kept it in storage. It's in generally good condition, but there is a tear that has been repaired. I've included photos of all the pages, so please take a look for details.
