It appears to be an "Asahi Mini Pet Heater (20w)" and a "Marukan Minimal Thermo." *Bulb not included. Approximately 6 years have passed since purchase. I haven't performed a test run because there's no bulb, but the items I bought around the same time are still working fine, so I hope this helps. The hook for the thermometer on the temperature controller is bent (see photo 7). There is also no hanging bracket.
