I've had this for a while now, but I've only worn it a handful of times. It has a back print, and I think it's in very good condition! The collar is like this from the beginning. The size is S, but it has a loose fit even on me, and I'm 172cm tall! I think the retail price was around 20,000 yen. Feel free to ask any questions in the comments!
