This is a set of a "Godzilla" themed medical record book and a special protective cover to keep it safe. Both items are brand new and unused (individually wrapped, unopened). The medical record book is A6 size and a thick 36-page type for long-term use. There is no space for pharmacy names or other information (such as stamp impressions) on the back.
