It can be worn either diagonally across the shoulder or hanging from the waist. The fabric is stretchy, so it should fit a travel-sized hand sanitizer bottle. For reference, I used to put a "GoJo 215ml" bottle in this pouch. It has been used and shows some marks (see photos). Please consider this with the understanding that it has been used, and those who are concerned about minor details should refrain from purchasing.
