This is a set of the two-part "Hajime" short story, which was not included in the collected volumes, by Takeshi Obata, the artist of Death Note and Bakuman. Original work by Otsuichi, art by Takeshi Obata. It was published in Weekly Shonen Jump before Death Note, in issues 5 (2003) and the combined issue 6/7. I cut it out from the Jump I bought at the time and stored it in a file. There are no missing pages, and there's no problem reading it, but the cut at the end of the first part is bad, and the page is about half detached.
