I forgot where I bought it. There are no noticeable stains or scratches on the lenses, but there is one scratch on the inside of the left temple (see photo 6). The prescription is -2.75 in both eyes. I had the prescription measured at an optician's, so I've included the document in the images. To cushion the impact during shipping, I will send it in a glasses case, but the case is very old.
