Yamaha has released a "HOKAZON" signature model for Shoichiro Hokazono, but this is a different mouthpiece. It is also said that it was included with the custom model euphonium "YEP-842S" for a short period, or that it was sold overseas as the "YAMAHA SH SPECIAL" model, but in any case, the production number seems to have been quite small, making it a phantom mouthpiece.
